The findings we are here to prevent
A deficiency is written about a room, not about a policy.
Two federal requirements do most of the work in this department. F584, under 42 CFR 483.10(i), covers the resident's right to a safe, clean, comfortable and homelike environment, and names housekeeping services and clean bed and bath linens in good condition. F880, under 42 CFR 483.80, covers infection prevention and control, and its linen provision requires personnel to handle, store, process and transport linens so as to prevent the spread of infection.
Those are cited to your facility, never to us. Which is exactly why they should be the contractor's problem before they are yours.
In practice the citation is never about the regulation. It is about a room with dust on the floor and a bathroom that was not cleaned, a linen closet with torn sheets, a soiled cart in a clean corridor, or a pest nobody logged. That is the level we manage at.
The state layer
The federal requirements are the floor.
Every state layers its own licensure rules on top, and those rules are not identical, do not share numbering, and get relocated between agencies more often than people expect. We work from the current rule set in the state your facility sits in, not from a national template written once and never revisited.

Room-level evidence, not a general assurance
Every room is signed for by the housekeeper and initialed by the supervisor, with refusals recorded in writing. When a surveyor asks when room 214 was last cleaned, that is a document, not a recollection.

Par levels by shift, monthly inventory by category, rag-out discipline, and recovery rounds with nursing for linen held on the units.

A pest log that is actually kept
Date, time, room, pest observed, person informed, housekeeper. Logged and escalated the same day, filed where you can produce it.

Isolation and terminal cleaning to your protocol
We follow the facility's infection prevention protocol, not a generic one, and we train to it before deployment and re-train when it changes.

Survey
Annual survey, complaint survey, and the revisit.
We prepare the environment ahead of the window, walk the building with your administrator and director of nursing, and hold the records where they can be produced in minutes. If an environmental or laundry deficiency is cited, we work to your plan of correction on your dates and document the correction in a form your surveyor can accept.
We do not promise a deficiency-free survey. Nobody honestly can, and the facility holds the license. What we commit to is that the environment, the linen and the records will not be the reason.
